The registration fee includes admission to all sessions for CCP2000, 
refreshment breaks, the conference reception, and one copy of the CCP2000 
Technical Digest. Full-time students must provide valid I.D. to receive 
the discounted registration. Forms received after the deadline of
October 31, 2000 will be charged the on-site fee.

To avoid duplicate charges, either mail or fax your registration 
form, NOT BOTH.

All forms received after the registration deadline of 
October 31, 2000 will be charged the higher fee.
    
Refund policy for registration: 
There will be a $50 service charge for processing refunds.
A letter requesting the refund should state the registrant's name 
and to whom the refund should be made payable. Requests for 
registration refunds must be received NO LATER THAN November 26,
